Become an adult volunteer in Scouting America! .
Annual Health & Medical Record (Parts A & B)
Required for all Pack activities including camping. Completed by parent/guardian. Renew annually.
Scouting Health Form Part C (Physical Exam)
Required for high-adventure and extended overnight activities (3+ nights). Completed by a licensed physician.
BSA Talent Release Form
Permission for photos/videos of your scout to be used in Pack communications and materials.
Youth Protection Training (YPT)
Required annually for ALL adult volunteers and den leaders. Takes about 90 minutes online — free.
